About The Position

Activate Care is hiring a community-focused Registered Nurse to travel to members’ homes and community settings across Massachusetts to complete in-person health assessments for a managed care health plan’s dually eligible members. This field-based role focuses on conducting timely, accurate assessments, documenting findings, and handing them off to the health plan’s care management team. It does not involve care plan development or ongoing case management. The role requires travel to members’ homes and community settings, with more than 70% travel within the assigned area. Visits may involve varied residential environments, changing weather, and extended periods of sitting, standing, or driving. Primary hours are during set daytime slots based on member availability; optional evening or weekend scheduling may depend on shift selection. Annual influenza vaccination is required during flu season (October 1–March 31), with proof due within 30 days of hire. Reasonable accommodation requests will be evaluated under applicable law. Activate Care’s Community Care Record platform, Care Link, supports coordination between healthcare and community organizations for populations with health-related social needs. Its Path Assist program uses a tech-enabled, evidence-based Community Health Worker intervention to address those needs, increase health confidence and self-efficacy, and reduce inappropriate healthcare spending.
