About The Position

Requirements

• Be at least 18 years of age and have a high school diploma or GED.
• Be currently registered as a State Registered Nurse Aide through the Kentucky Board of Nursing.
• Provide proof of completion of a recognized CNA training program.
• Have at least one year of experience working with frail and elderly people, or receive proper training upon hire.
• Complete a food handling permit upon hire.
• Possess and maintain current CPR certification.
• Be self-directed and able to work with little direct supervision.
• Demonstrate the knowledge and skills necessary to provide care to and communicate with adults.
• Be able to read and interpret safety rules, operating and maintenance instructions, and procedure manuals.
• Be able to write routine reports and correspondence.
• Be able to speak effectively before groups of participants or team members.
• Use common-sense understanding to carry out written, oral, or diagrammatic instructions and solve problems involving several concrete variables in standardized situations.
• Accommodate daily schedule changes arising from staffing, participant condition and needs, and new participants.
• Hold a valid driver’s license and have an automobile in good working order, insured according to state and/or organizational requirements, with personal auto liability coverage of $100,000/$300,000 in bodily injury coverage.
• Demonstrate tact, patience, and good personal hygiene.
• Comply with Bluegrass Care Navigators employee health, infectious disease, and vaccination policies.

Responsibilities

• Assist participants with bathing, dressing, toileting, meal preparation, and feeding.
• Monitor participants’ vital signs and document intake and output.
• Perform routine housekeeping tasks and chore services to help maintain a clean, safe, and supportive environment.
• Transport participants and assist with simple errands.
• Observe and report changes in participants’ conditions to appropriate clinical staff.
• Adapt care to participant status and needs as directed.
• Follow emergency and evacuation procedures.
• Collaborate with PACE staff and members of the Interdisciplinary Team.
