About The Position

Transition Home Healthcare is seeking a Certified Nursing Assistant (CNA) to provide home health care in the North, Northeast, and Northwest service areas. The CNA provides care in coordination with the assigned nursing care team and supports patients with meals, feeding, and activities of daily living. The role includes maintaining patient records, documenting clinical notes, communicating condition changes to the case manager and care team, and educating patients and family members. The available shift is PRN.

Requirements

• CNA license in the state of Texas.
• One year of home health experience.
• Ability to lift up to 50 pounds.
• Ability to work occasional weekends, holidays, and evenings.
• Strong communication and people skills.

Nice To Haves

• Experience with HCHB EMR; preferred but not required.

Responsibilities

• Provide care in coordination with the assigned nursing care team.
• Maintain patient records, including changes in doctor’s orders, diet, or personal living activities.
• Assist patients with meals, feeding, and basic activities of daily living.
• Deliver weekly reports to the case manager and review important changes in patient conditions.
• Educate patients and family members.
• Prepare timely, complete, and accurate clinical notes.
• Communicate effectively with healthcare providers and other care team members.
