About The Position
Transition Home Healthcare is seeking a Certified Nursing Assistant to provide home health care in coordination with an assigned nursing care team. The CNA supports patients with meals, feeding, and basic activities of daily living; maintains patient records and clinical notes; reports changes in patient condition to the case manager; educates patients and family members; and communicates with healthcare providers and other care team members. Coverage areas are North, Northeast, and Northwest. The available shift is PRN.
Requirements
- CNA license in the state of Texas.
- One year of home health experience.
- Ability to lift up to 50 pounds.
- Ability to work occasional weekends, holidays, and evenings.
- Strong communication and people skills.
Nice To Haves
- Experience with HCHB EMR (preferred but not required).
Responsibilities
- Provide exceptional care in coordination with the assigned nursing care team.
- Maintain patient records, including changes in doctor’s orders, diet, or personal living activities.
- Assist with patient meals, feeding, and basic activities of daily living.
- Deliver weekly reports to the case manager and review important changes in patient conditions.
- Educate patients and family members.
- Prepare timely, complete, and accurate clinical notes.
- Communicate effectively with healthcare providers and other care team members.
