About The Position

The Certified Nurse Aide (CNA) will work in the Adult Day Health Care department. This role involves overseeing registrants during daily scheduled activities, assisting them with Activities of Daily Living (ADLs), and providing personal care. The CNA will also assist registrants in moving to different areas within the facility and potentially transporting them using facility vehicles. This position requires adherence to ethical standards, facility policies, and documentation of registrant care.

Requirements

  • High school diploma or equivalent.
  • Successful completion of a Certified Nurse Aide training program or enrollment in an accredited licensed nursing program.
  • Valid New York State Class 5 or "D" license with a clean driving record.
  • Ability to lift or bear weight up to 100 lbs when transferring registrants.
  • Ability to follow written and verbal instructions.
  • Completion of mandatory inservices and departmental inservices.
  • Adherence to the facility's confidentiality statement.

Nice To Haves

  • Geriatric experience preferred.

Responsibilities

  • Oversee registrants during daily scheduled activities and assist with ADLs.
  • Assist registrants with arrival, departure, and movement to specific areas (e.g., OT, PT, dining room, activities, dental, beauty-barber shop).
  • Initiate activity programs in conjunction with supervisor and/or recreation leader.
  • Provide personal care assistance, such as baths/showers.
  • Lift or bear weight up to 100 lbs when transferring registrants.
  • Travel to and from registrants' homes to pick up/drop off, if necessary (door-to-door assistance).
  • May drive facility vehicles to transport registrants.
  • Follow written and verbal instructions.
  • Complete mandatory and departmental inservices.
  • Review registrant care plans per schedule.
  • Document necessary information in registrant records.
  • Observe and report changes in registrant behavior, attitudes, and/or physical capabilities to the nurse and/or supervisor.
  • Perform nursing aide duties on other units (e.g., rehab or SNF houses) as needed.
  • Complete assigned documentation (medical flow sheets, vital signs, etc.).
  • Perform duties according to the highest ethical and moral standards and in adherence with the facility's corporate compliance plan and all applicable governing laws.
  • Adhere to the facility's confidentiality statement in accessing and safeguarding protected health information (PHI) of residents and employees.
  • Perform any other duties as assigned by Program Director or designee.
