About The Position

Requirements

• Current Ohio Physical Therapist license.
• At least one year of recent work experience as a Physical Therapist.
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by a school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills.

Nice To Haves

• New graduates are welcome; a mentorship program is available.
• Prior experience with electronic medical records (EMR) is preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, including assessments of joint mobility and range, transfer status, ambulation, muscle strength and endurance, balance, and safety.
• Provide integumentary protective interventions to prevent wounds and promote wound repair using evidence-based dressings and topical agents.
• Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance training, and other interventions.
• Continually assess and revise the physical therapy care plan, and participate with nursing in multidisciplinary care planning as appropriate.
• Educate and instruct clients, family members, or other representatives in rehabilitative care and activities that promote health, safety, and independent living.
• Document observations, interventions, evaluations, and services provided using a tablet.
