Weekends Physical Therapist / PT (Home Health)
About The Position
Mission Healthcare is hiring a Physical Therapist to provide home health care in the Grover Beach, Santa Maria, and Lompoc territories. The role involves assessing patients, developing individualized treatment plans, delivering therapeutic care, and supporting patients and caregivers with education and self-care strategies. The Physical Therapist works collaboratively with an interdisciplinary team and maintains accurate documentation of patient progress and treatment plans. This is a full-time, 32-hour-per-week role scheduled Thursday through Sunday. The stated pay range is $60–$72 per hour. Requirements
- Active Physical Therapist license in the practicing state.
- Master's or Doctorate degree in Physical Therapy.
- Minimum of 1 year of experience as a Physical Therapist.
- Current BLS and hands-on CPR certification.
- Valid driver's license and reliable, insured vehicle.

- Home health experience is strongly preferred.
Responsibilities
- Evaluate and assess patients' physical conditions and develop individualized treatment plans.
- Implement therapeutic exercises and techniques to relieve pain and improve mobility, strength, and coordination.
- Educate patients and caregivers on exercises, safety precautions, and self-care strategies.
- Collaborate with an interdisciplinary team to provide holistic, patient-centered care.
- Maintain accurate and timely documentation of patient progress and treatment plans.
