About The Position

Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. The Physical Therapist provides skilled physical therapy to patients in their homes to promote mobility, restore function, improve strength and balance, and enhance safety and independence. The role works collaboratively with patients, caregivers, and an interdisciplinary care team to support recovery, prevent hospitalizations, and improve quality of life in the home setting. The posting offers a $5,000 sign-on bonus for external candidates.

Requirements

• Graduate from an accredited physical therapy educational program as set by the Commission on Accreditation of Physical Therapy Education (CAPTE).
• Hold a current license to practice physical therapy in the state assigned.
• Have a driver's license and access to reliable transportation.
• Hold current CPR certification.
• Have at least 1 year of related physical therapy experience when entering home health care to ensure sufficient knowledge and skills to safely provide services.
• If experience is less than 1 year, obtain approval from regional clinical leaders and receive appropriate mentoring and/or participate in a residency program.

Responsibilities

• Help patients maintain, restore, and improve movement, activity, and function.
• Provide interventions, conduct reexaminations at least every 30 days, modify interventions as necessary, and perform discharge planning.
• Perform other duties as assigned.
