About The Position
HealthPro Heritage is seeking a Physical Therapist to provide home health services to adult patients in Groton, Connecticut, and surrounding areas. This is a full-time position with a flexible caseload supporting the organization’s partnership with Masonicare Home Health. The estimated total annual pay range is $99,840–$116,480, with a point base rate of $60–$70 per point and a $10,000 sign-on bonus. The therapist evaluates patients’ physical conditions and provides treatments intended to improve function and alleviate pain. The role includes educating patients and families, documenting care, and collaborating with other healthcare providers. Requirements
- Degree in Physical Therapy from an accredited institution.
- Valid state licensure as a Physical Therapist, or license eligibility.
- Strong clinical assessment and diagnostic skills.
- Proficiency in developing and implementing effective treatment plans.
- Excellent communication and interpersonal skills with a patient-centered approach.
- Ability to lift and assist patients as needed.
- Ability to stand for extended periods and perform physical tasks related to therapy.
Responsibilities
- Evaluate patients’ physical conditions, including mobility, strength, and flexibility.
- Provide therapeutic exercises, manual therapy, and other treatments to improve functionality and alleviate pain.
- Educate patients and families about exercises, posture, and body mechanics to support recovery and injury prevention, and provide guidance on home routines and lifestyle changes.
- Maintain accurate records of evaluations, progress, and treatment plans in compliance with legal and ethical standards.
- Collaborate with healthcare providers to support effective patient care and participate in multidisciplinary team meetings as needed.
