About The Position

Elara Caring is seeking a compassionate Physical Therapist to provide home-based physical therapy services. The role involves evaluating patients, developing individualized treatment plans, monitoring progress, and adjusting care as needed. The therapist collaborates with physicians, caregivers, and the interdisciplinary care team, completes timely and accurate clinical documentation, and supervises therapy assistants and support staff as appropriate. The position requires travel between patient homes and assigned work locations.

Requirements

• Graduate of an American Physical Therapy Association (APTA)-approved physical therapy program.
• Current, unrestricted physical therapist license valid in the state of work.
• At least one year of experience in a healthcare facility.
• Valid driver’s license and acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide physical therapy services in patients’ homes.
• Monitor patient progress and adjust care plans as needed.
• Collaborate with physicians, caregivers, and the interdisciplinary care team.
• Complete timely and accurate clinical documentation.
• Supervise therapy assistants and support staff, as appropriate.
