About The Position

Adoration Home Health is seeking a dedicated Home Health Physical Therapist to provide expert, patient-centered care in the Monroeville, Alabama coverage area. The role offers one-to-one patient care, flexible scheduling, independent work with team support, and the opportunity to make a meaningful impact on patients and families. The position is scheduled PRN.

The Physical Therapist evaluates and treats patients according to physician orders, develops measurable goals and treatment plans, and works to help patients reach their maximum independence and function. The role also involves coordinating with the treatment team, physicians, patients, and families; maintaining timely service documentation; and supervising Physical Therapist Assistants and Physical Therapy Aides in accordance with the state practice act.

Requirements

• Bachelor’s degree in Physical Therapy from a college or university with an accredited Physical Therapy program.
• At least two years of appropriate experience as a physical therapist.
• Valid license in the state of practice.
• Current CPR certification.

Nice To Haves

• Community or hospice experience is preferred.
• New physical therapy graduates may be considered in select markets based on program availability.

Responsibilities

• Perform physical therapy evaluations and treatments based on physician orders, including recommending and procuring adaptive equipment.
• Establish measurable goals and develop treatment plans to help patients achieve their maximum level of independence and function.
• Maintain communication among the treatment team, physician, patient, and family members as applicable.
• Maintain thorough and timely documentation of services provided to support proper billing.
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with the state practice act.
