About The Position

Adoration Home Health is seeking a dedicated Physical Therapist to provide patient-centered home health care in Wheeling, West Virginia. The role serves the Wheeling coverage area and is scheduled PRN. The therapist provides one-to-one care in patients’ homes, works independently with team support, and collaborates with patients, families, physicians, and other members of the treatment team.

Requirements

• Bachelor’s degree in Physical Therapy from a college or university with an accredited Physical Therapy program.
• At least 2 years of appropriate experience as a physical therapist.
• Valid license in the state of practice.
• Current CPR certification.

Nice To Haves

• Community or hospice experience is preferred.
• New physical therapy graduates may be considered in select markets based on program availability.

Responsibilities

• Evaluate and treat patients according to physician orders, including recommending and procuring adaptive equipment.
• Establish measurable goals and develop treatment plans to help patients reach their maximum level of independence and function.
• Maintain communication with the treatment team, physician, patient, and family members as applicable.
• Complete thorough and timely documentation of services to support proper billing.
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and related activities in accordance with the state practice act.
