About The Position

The Oncology Registered Nurse (RN) Care Coordinator plays a vital role in the Cancer Care Center at Advocate Illinois Masonic Medical Center. This full-time position focuses on providing direct and indirect patient care to individuals with cancer-related health concerns, aiming to promote positive clinical outcomes, patient satisfaction, and overall well-being. The role involves comprehensive patient assessment, planning, implementation, and evaluation of care, as well as patient education, emotional support, and coordination of services across various healthcare settings. Additionally, the RN Care Coordinator contributes to community education and professional development to stay abreast of industry innovations and standards in oncology care.

Requirements

  • Current RN license in the state of Illinois.
  • CPR certification.
  • B.S.N.
  • 3-5 years focused experience in clinical areas.
  • Board Certification by 12/31/2015 or within two years of hire.
  • Good communication skills.
  • Ability to access, plan, intervene, and evaluate the physical, emotional, and educational needs of patients.
  • Knowledge of computer operations and applications.
  • Ability to lift up to 35 pounds without assistance.
  • Ability to work under stressful conditions and in difficult situations.
  • Ability to work with patients in a fast-paced environment with established time constraints.

Responsibilities

  • Assess, plan, implement, and evaluate care of women and men with cancer related health concerns.
  • Interview and assess new patients for physicians.
  • Assist physicians with clinic visits and procedures as needed.
  • Assist patients and staff in the appointment process to expedite timely appointments.
  • Provide patients with appropriate education related to prevention, diagnosis, tests, surgery, chemotherapy, and radiation therapy.
  • Teach patients about the importance of screenings such as mammograms, colonoscopy, etc.
  • Coordinate testing and expedite results reporting, utilizing the QCL and other tools in the EMR.
  • Provide emotional support to patients.
  • Assure continuity of care by interfacing with day surgery, in-patient units, and out-patient facilities.
  • Maintain the QCL for the MD and facilitate patient flow through the continuum of care in partnership with the physician and other members of the health care team.
  • Follow-up with physicians on all abnormal testing in a timely fashion and document in the EMR.
  • Document all education, assessments, and care given in the EMR.
  • Identify and collect appropriate Quality Improvement data.
  • Follow-up with patients in a timely and professional manner and document in the EMR.
  • Act as a resource for the hospital in promoting cancer awareness, education, and screening tools.
  • Act as a resource for patients and other employees.
  • Use effective listening and questioning techniques in a professional and empathetic manner, demonstrating sensitivity to patients and families.
  • Develop educational tools for patients.
  • Identify, develop, and participate in all hospital and community programs related to cancer screening, diagnosis, and cancer-related health issues.
  • Develop education programs for cancer screenings and awareness.
  • Participate in AIMMC Cancer Committee activities.
  • Under the direction of the Radiation Oncology Coordinator and Cancer Center leadership, work with all hospital departments to provide seamless care for patients recently diagnosed with cancer.
  • Teach other hospital departments and employees about cancer education, screening tools, and treatment options.
  • Develop educational programs for the public.
  • Work with the Wellness Communities of the Chicagoland area to support their efforts.
  • Responsible for continued self-growth and education in areas related to changes in cancer detection, education, and treatment options.
  • Present a professional image and courteous manner representing AIMMC to the public.
  • Participate in unit and hospital activities to promote service excellence.
  • Promote team behaviors and act as a role model for others.
