Responsibilities

• Serve as the first point of contact for clients (current and potential), industry partners, and care providers, providing excellent customer service.
• Handle phone inquiries from potential clients, offering accurate information about services and fees, and collecting relevant details for client care plans.
• Monitor the Dashboard daily and follow up with caregivers and clients.
• Participate in an 'On call' rotation every other month.
• Input care plans, ensure adequate staffing, and review relevant information with assigned caregiver(s) before care begins.
• Conduct monthly client calls as per policy and document interactions in client files.
• Update care plans as necessary, upload all monthly reports to client profiles, communicate changes to caregiver(s), and document these changes.
• Facilitate optimal matches between caregivers and clients.
• Ensure adherence to care plans and accurate documentation of all care plan tasks.
• Participate in staff training and operational meetings.
• Perform clerical duties such as filing, uploading, copying, and faxing.
• Undertake other job-related duties as assigned.
