Requirements

• Bachelor’s degree in related field and two years’ experience in a directly related role serving a community-based or vulnerable populations; OR, any equivalent combination of education, training and experience which demonstrates the ability to perform the duties of the position.
• Must obtain an Arizona driver's license by date of hire and maintain it throughout employment.
• Final job offer will be contingent upon a satisfactory 5-year Arizona Motor Vehicle Report.
• Fingerprint clearance may be required.
• Working knowledge of the challenges faced by people reentering society after incarceration and ability to develop effective programs to address these challenges
• Working knowledge of programs and services provided by governmental and non-governmental organizations that work to improve the health of the community
• Working knowledge of basic public health issues, methods, and principles
• Working knowledge of social determinants of health and engaging vulnerable populations
• Working knowledge of helping others improve their health status and quality of life
• Working knowledge of relevant public health training and educational resources to support public health goals and improve individual outcomes
• Working knowledge of business English, spelling and arithmetic, punctuation and grammar
• Working knowledge of office practices and procedures
• Working knowledge of criminal justice systems
• Working knowledge of the prevalence of behavioral health and substance use and related needs of individuals
• Working knowledge of principles and practices of crisis intervention and trauma informed care

• EPICS-II (Effective Practices in Correctional Settings) training or certification.
• Experience working as a Community Health Worker, case manager, peer support specialist, or in a similar role serving justice-involved populations.
• Experience providing coordination or training support to staff or teams in a community-based setting.

Responsibilities

• Performs the duties of Community Health Care Worker I and II
• Serves as the Community Health Worker team Lead
• Coordinates re-entry services for clients providing resource navigation services to improve client outcomes
• Leads case staffing meetings for program clients with program staff and partners
• Supervises and evaluates the work of Community Health Workers and facilitates training as needed
• Conducts training and outreach efforts in the community
• Identifies culturally appropriate training modalities for delivery to clients and community in response to identified community need
• Responsible for assigning and reviewing the work of others
• May be responsible for the development of additional trainings as required for the population(s) served
• Oversees Community Health Worker schedules, work plans, performance evaluations and disciplinary actions
• Acts as department liaison with various agencies providing resources
• Completes Community Health Worker client chart review and audits for PTC clients
• Performs related duties and fulfills responsibilities as assigned
