Pathways to Community is a collaborative partnership between the Coconino County Sheriff’s Office and Coconino County Health and Human Services (CCHHS) that provides reentry support to individuals transitioning from the Coconino County Detention Facility back into the community. The Community Health Worker (CHW) Lead serves as the lead for Community Health Worker services and is responsible for coordinating daily CHW operations, supervising and supporting CHW staff, and ensuring timely engagement with individuals both prior to and following release from custody. This position reports to the Pathways to Community Manager and plays a critical role in fostering a collaborative, trauma-informed environment that supports successful reentry outcomes through connection to behavioral health, healthcare, housing, transportation, employment, and other community-based resources. Under general direction, is responsible for work of significant difficulty in performing professional community re-entry and community health work for the Pathways to Community Program, performing related work as assigned.
