About The Position

Requirements

• High school diploma or equivalent.
• A minimum of one year of combined experience in health, education, and social services.
• Basic understanding of health conditions such as HIV/AIDS, diabetes, COPD, asthma, CHF, and CVD.

Nice To Haves

• Bachelor’s degree in a human services field is preferred but not required.
• Fluency in Spanish is preferred.

Responsibilities

• Communicate program purposes to clients and explain the potential impact of the program.
• Conduct intake interviews and enroll or refer individuals into community programs.
• Assist health care providers involved in clients’ care to ensure clients understand their care plans.
• Support the development, implementation, and execution of care plans.
• Complete monthly reporting and maintain electronic client files, including updated releases of information, assessments, and other medical documents.
• Travel extensively to client homes, community locations, agencies, and other outreach destinations.
• Assess, plan, implement, monitor, and evaluate actions to meet clients’ health and human services needs.
• Coordinate and provide safe, timely, effective, efficient, equitable, and client-centered support.
