About The Position

Requirements

• Bachelor’s (BSW) or Master’s (MSW) degree in Social Work from an accredited school is preferred.
• At least two years of experience in clinical medical case management in acute care, extended care, or rehabilitation is preferred.
• Current BLS Healthcare Provider card.
• Professional knowledge of psychosocial needs and the effects of disease, treatment, and survivorship.
• Analytical skills to evaluate psychosocial and medical needs and develop and implement continuing care plans.
• Ability to handle sensitive and highly confidential issues.
• Ability to comply with correctional facility security and safety requirements.

Responsibilities

• Maintain upward communication with department managers.
• Provide inmates with instructions and information about their plans of care.
• Collaborate with healthcare team members and outside agencies.
• Screen and assess patients needing counseling, support, or discharge planning.
• Assign acuity levels and prioritize the caseload based on acuity.
• Assess psychosocial needs by reviewing and interpreting medical records and other documents, and document assessments in a timely manner.
• Plan and provide care with consideration for the patient’s age, development, understanding, and ability to follow instructions.
• Provide privacy for patient and family interviews or consultations whenever possible.
• Provide lists of available referral agencies and support patients or families in choosing services for continuity of care.
• Act as a liaison among professional staff, patients, and outside agencies.
• Educate patients, families, and caregivers about community resources and entitlements to support informed decisions about resources and long-range care needs.
• Provide patients and families with information about DNR orders and advance directives.
• Provide information needed for insurance authorization to the department manager or referred agency.
• Explain federal, state, and third-party payer regulations and procedures related to post-care to patients or families.
• Advocate for patient rights, help remove barriers to quality care, address service gaps, and support access to legal protections and needed policy or program changes.
• Assist patients and families with coping with or adjusting to illness, disability, or death.
• Provide consultation, appropriate action, or referrals related to adjustment, financial, or disability needs.
• Assist with duties related to the MAT program.
• Comply with facility security policies, including access control, staff identification, patient movement, contraband prevention, and emergency procedures.
• Coordinate with correctional officers regarding clinic access, patient escorts, medical appointments, and security support during clinical encounters.
• Remain aware of the correctional environment and promptly report potential threats, unsafe conditions, or security concerns.
• Maintain security and accountability of medications, sharps, clinical equipment, and medical supplies.
• Maintain professional boundaries and avoid unauthorized relationships, communications, or preferential treatment involving incarcerated individuals.
• Balance clinical responsibilities with security requirements while advocating for timely access to medically necessary care.
• Participate in required security orientation, safety training, and emergency preparedness activities.
