About The Position
The Social Worker provides coordinated care in patients’ homes to patients of all age groups. The role includes conducting psychosocial assessments and analysis, counseling patients and families, and making referrals to address their needs. The Social Worker completes a comprehensive assessment within five days of a patient’s election of hospice care, supports patients and families with care planning and end-of-life needs, and collaborates with the interdisciplinary group (IDG) to coordinate patient care. The position also promotes respect for patients, their privacy, and their property.
Requirements
- Bachelor’s degree in Social Work from an accredited school.
- One or more years of hospice experience.
- Valid driver’s license.
- Valid auto insurance.
- AHA CPR certification.

- HomeCare HomeBase experience preferred.
Responsibilities
- Complete a comprehensive assessment within five days of a patient’s election of hospice care to identify psychosocial, financial, environmental, and community needs, documenting findings in the plan of care, clinical records, IDG meetings, and community resource referrals.
- Assist with advance directives, funeral planning, discharge planning, family counseling, and changes in the level of care as indicated by patient or family needs.
- Use effective interpersonal relations and communication skills to achieve desirable outcomes.
- Provide patient and family or caregiver counseling about end-of-life issues, death, and grief.
- Attend interdisciplinary team meetings and participate in patient care planning, coordinating and collaborating with the IDG to promote coordination of care.
- Promote an environment that respects patients, their privacy, and their property.
