About The Position

The Social Worker delivers social work services related to chronic disease management, including care coordination, planning, concrete and supportive counseling, and assistance with entitlements and other community resources. The role also provides consultation, education, and other services requested by clinicians within the Department, in the Hospital, or in the community. Services are provided in accordance with National Association of Social Workers (NASW) and Case Management Standards, as well as the Hospital’s mission and goals.

Requirements

• Master’s Degree in Social Work.
• Previous hospital work experience (required).
• New York State Licensed Master Social Worker (LMSW) license.
• Screen certification is required for Social Workers in In-Patient Units; new hires have 90 days to obtain it, effective 1/1/2023.
• Broad knowledge of community resources and social work and case management regulatory requirements.
• Ability to handle high-stress situations.

Nice To Haves

• Discharge planning experience.
• Knowledge of computer applications, including word processing, spreadsheets, and clinical application software.
• Knowledge of evidence-based therapeutic techniques.
