PurposeCare is seeking a qualified, dependable, honest, and reliable Medical Social Worker (MSW) to support the needs of our clients. PurposeCare is a leading provider of home care and home health services in the Midwest, serving over 3,000 clients monthly. Requirements

  • Master's Degree in Social Work is required
  • At least 1 year of social work experience in the healthcare field preferred
  • Must possess a valid driver's license

Responsibilities

  • Assessment of psychological, emotional and/or economic factors affecting patient's limitations and potential for a lack of improvement
  • Collaborates with the physician and other members of the health team to develop a Plan of Care
  • Comply with all aspects of the company's compliance and HIPPA regulations
