About The Position

DaVita is hiring a Social Worker to serve as a vital member of each patient’s core care team in an outpatient dialysis setting. The Social Worker supports and advocates for patients as they adjust to life on dialysis, helps them understand their rights and responsibilities, and guides them in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease. The role emphasizes building meaningful, long-term relationships with patients and families and contributing to a positive, supportive team environment. Paid training is provided. Compensation is $82,200–$130,000 per year and depends on qualifications, skills, competencies, and experience.

Requirements

Master’s degree in Social Work (MSW), with a specialization in clinical practice.
Active Associate Clinical Social Worker (ASW) status is required.
Registration with the California Board of Behavioral Sciences (BBS) is required.
Meet all state-required regulations to practice in the dialysis setting.
Demonstrated knowledge of government and private insurance programs.
Basic computer skills in MS Word, Excel, PowerPoint, and Outlook.
Functional proficiency with DaVita-specific applications within 60 days.

Responsibilities

Support and advocate for patients adjusting to life on dialysis.
