About The Position

The Social Worker provides social services to residents and their families to help them cope with the impact of illness and extended care placement. The role participates in resident care management and helps ensure residents’ needs are met through social service programming and appropriate referrals to outside agencies and community resources. This is a full-time position at OakPointe Senior Care and Rehab in Detroit, Michigan.

Requirements

One to two years of supervised social work experience in a health care setting working directly with individuals.
Excellent customer service skills and commitment to delivering excellent, passionate, and innovative care.
Ability to work as a team player and support team members to get the job done.

Nice To Haves

A degree in Social Work is preferred but not required.
A Social Work license is preferred but not required.
Nursing home experience is preferred.

Responsibilities

Identify residents’ medically related social service needs and help ensure those needs are addressed by the appropriate discipline.
Maintain contact with families, with resident permission, to report changes in behavior and psychosocial functioning, discuss discharge planning, and encourage participation in care planning.
Assist residents or responsible parties with financial and legal matters, including drafting guardianship or conservatorship petitions and helping arrange prepaid funeral plans.
Coordinate discharge planning, including community congregate-living waitlists, home-care intake for residents returning home, and transfers to other facilities.
Use assessment and care planning to identify ways to support residents’ individual needs and preferences, customary routines, concerns, and choices.
