About The Position

Best Home Health Providers Inc. is a rapidly growing provider serving patients throughout the Bay Area and beyond. Its mission is to support patients transitioning home from medical facilities by providing expert guidance during recovery. The Medical Social Worker will shape home-based patient care, lead and mentor a team of social workers, and help establish standards for compassionate, high-quality service. The position is per diem, with compensation of $100–$120 per visit.

Requirements

Master of Social Work (MSW) from an accredited program.
Active California LCSW license.
At least 3 years of management experience; home care experience is preferred.
Strong knowledge of healthcare regulations, ACHC standards, and business operations.
Excellent communication skills and ability to build trust with patients, families, staff, and the community.
Self-directed, organized, and flexible approach, with initiative and ability to work well with others.

Responsibilities

Lead daily operations of the social services team, including policies, procedures, and care coordination with other clinical providers.
Recruit, onboard, and mentor social workers by providing clinical supervision, observing practice, and supporting professional growth.
Oversee assessment, planning, and evaluation of social services care, and ensure clinical documentation meets state, federal, and ACHC standards.
Partner with the Executive Director on the annual department budget and resource planning.
Build knowledge of community resources and connect patients and referral sources with appropriate support.
Plan in-service training and continuing education for the team.
Contribute to quality improvement initiatives and identify ways to better serve patients.
Perform other duties as required for patient services.
