Medical Social Worker
About The Position
Guardian Angel Home Care is seeking an experienced Medical Social Worker (MSW) to join its home health team on a PRN/per diem basis. This field-based role involves traveling to patients’ homes throughout the assigned service area. It offers flexible scheduling, the ability to accept visits based on availability, high per-visit compensation, and possible hourly compensation depending on the assignment. Additional assignments may become available as patient census and coverage needs grow. Candidates must have a Master’s Degree in Social Work and a full, active, unrestricted social work license. Limited, temporary, associate-level, provisional, or otherwise restricted licenses are not accepted. The role also requires a valid driver’s license, a reliable personal vehicle, and current auto insurance listing the applicant’s name.
Requirements
- Master’s Degree in Social Work (MSW).

- Strong knowledge of community resources.
- Excellent communication and documentation skills.
- Ability to work independently in patients’ homes.

Responsibilities
- Complete psychosocial assessments.
- Evaluate patient and family needs.
- Provide counseling and emotional support.
- Connect patients with community resources and support programs.
- Assist with financial, transportation, housing, and long-term care resources.
- Support patients and families as they adjust to illness or disability.
- Participate in interdisciplinary care planning.
- Coordinate with nurses, therapists, physicians, and other clinical staff.
- Document visits and interventions accurately and in a timely manner.
- Travel to patients’ homes throughout the assigned service area.
