About The Position

AdvisaCare Home Health and Hospice is seeking a compassionate full-time Master of Social Work (MSW) to serve patients and families in the Genesee and Lapeer County areas. The Troy location is the role’s parent agency. The social worker supports patients with life-limiting illnesses and their families through assessment, counseling, care planning, advocacy, referrals, and bereavement services.

Requirements

Master of Social Work degree from a school accredited by the Council on Social Work Education (CSWE).
Valid driver’s license and car insurance.
Reliable transportation.

Nice To Haves

One year of home health care experience preferred.
Hospice experience preferred.

Responsibilities

Perform initial psychosocial, emotional, spiritual, and bereavement assessments and assist with developing and implementing goal-directed care plans.
Provide care under the direction of a physician.
Participate in interdisciplinary team (IDT) conferences, sharing information about psychosocial, emotional, spiritual, physical, and financial conditions affecting patients and families.
Document patient and family visits, telephone contacts, and referral actions accurately and promptly in patient records.
Reassess patient and family needs and provide counseling as required.
Provide crisis intervention and individual or family counseling when indicated.
Actively advocate for the patient and family unit.
Develop and maintain relationships with community agencies and services to promote cooperation and facilitate referrals.
Provide ongoing counseling to patients and families regarding death and dying as needed.
Participate in the Bereavement Team as assigned.
Follow all Hospice policies.
