About The Position

AdvisaCare Home Health and Hospice is seeking a compassionate, full-time Master of Social Work (MSW) Social Worker to support patients with life-limiting illnesses and their families. The role provides psychosocial, emotional, spiritual, and bereavement assessments and counseling, supports goal-directed care planning, and advocates for patients and families throughout the hospice journey. The Social Worker provides care under physician direction, participates in interdisciplinary team conferences, documents patient and family contacts, reassesses needs, and coordinates with community agencies and services. The position may also include crisis intervention, bereavement team participation, and counseling related to death and dying.

Requirements

MSW degree from a school of social work accredited by the Council on Social Work Education (CSWE).
Valid driver's license, car insurance, and reliable transportation.

Nice To Haves

One year of experience in home health care preferred.
Hospice experience preferred.

Responsibilities

Perform initial psychosocial, emotional, spiritual, and bereavement assessments and help develop and implement goal-directed care plans.
Provide care under the direction of a physician.
Participate in interdisciplinary team conferences and share information about psychosocial, emotional, spiritual, physical, and financial conditions affecting patients and families.
Document patient and family visits, telephone contacts, and referral actions accurately and promptly in patient records.
Reassess patient and family needs and provide counseling as required.
Provide crisis intervention and individual or family counseling when indicated.
Act as a care advocate for the patient and family unit.
Develop and maintain relationships with community agencies and services to promote cooperation and facilitate referrals.
Provide ongoing counseling to patients and families about death and dying as needed.
Participate in the Bereavement Team as assigned.
Adhere to hospice policies.
