Per Diem/Part Time Hospice MSW Social Worker

About The Position

AdvisaCare is a home health and hospice provider focused on personalized in-home care and compassionate support for patients and families. The Hospice MSW Social Worker helps guide patients and families through the hospice journey by providing compassionate advocacy and psychosocial, emotional, spiritual, and practical support. The role provides care under physician direction and works with the interdisciplinary team (IDT), patients, families, and community agencies to support individualized care.

Requirements

MSW degree from a school of social work accredited by the Council on Social Work Education (CSWE).
Active LMSW or LLMSW license in Michigan.
Prior experience working with patients and families.
Valid driver's license, car insurance, and reliable transportation.
Eligible to work in the United States.

Responsibilities

Perform initial psychosocial, emotional, spiritual, and bereavement assessments and help develop and implement goal-directed care plans.
Provide care under physician direction and participate in IDT conferences, sharing observations and information about conditions affecting patients and families.
Document patient and family visits, telephone contacts, and referrals accurately and promptly in the patient record.
Reassess patient and family needs and provide counseling as required.
Provide crisis intervention and individual or family counseling when indicated.
Act as a care advocate for the patient and family unit.
Maintain contact with community agencies and services to promote interagency cooperation and facilitate referrals.
Provide ongoing counseling about death and dying to patients and families as needed.
Participate in the Bereavement Team as assigned.
Adhere to hospice policies.
