About The Position

Requirements

• Current registered nurse license in the State of Florida.
• At least one year of nursing experience in an acute care setting is preferred.
• Strong ethical standards and an appropriate professional demeanor.
• Sound critical-thinking, time-management, technical, and analytical skills, and good judgment.
• Well-organized, self-directed, and a team player.
• Reliable transportation to perform duties in a timely and appropriate manner.
• Proof of automobile insurance and a valid Florida driver's license.
• AHCA Level 2 fingerprint screening for eligibility through the AHCA Florida Care Provider Background Screening Clearinghouse, as required for Gulfside Healthcare Services positions except some Thrift Shoppe positions.

Responsibilities

• Case manage the needs of patients in the IPU during the shift.
• Assess the ongoing physical, psychological, social, and spiritual needs of patients and families and initiate appropriate interventions.
• Provide or oversee supervision of LPNs, CNAs, and other assigned staff.
• Take independent action to identify problems and proactively identify and resolve potential issues.
• Support organizational policies and procedures.
• Assist with orientation of new staff members.
• Serve as a resource person by mentoring and guiding coworkers and additional staff.
• Maintain positive working relationships with team members across disciplines.
• Address patient and family complaints and concerns.
• Create and promote initiatives to develop and strengthen relationships with care facility partners.
• Educate the community about hospice care, philosophy, and concept through individual consultation.
• Communicate regularly with the Clinical Care Manager - IPU and continuously assess areas for risk management.
• Stay current with organizational and departmental communications via email, checking email at the start of each shift.
• Communicate, collaborate, and coordinate with colleagues and management about nursing processes and services.
• Create and implement nursing care plans, review them daily, and update them as needed.
• Consider financial implications when providing cost-effective nursing care.
• Maintain professional standards of nursing practice.
• Complete required documentation before leaving the scheduled shift.
• Participate actively in the Interdisciplinary Group and communicate patient needs to other disciplines when requested.
• Collaborate daily with the team physician regarding patient status.
• Work closely with the Psychosocial team to determine an appropriate discharge plan for each patient.
• Perform other duties as requested by the Clinical Care Manager - IPU.
