About The Position

Requirements

• High school diploma or general education degree (GED).
• Achieve Community Health Worker certification requirements within six months of employment and maintain certification while employed.
• Excellent interpersonal communication and problem-solving skills.
• Ability to intervene and promote reconciliation, compromise, and positive outcomes in difficult interactions.
• Bilingual in English and Spanish.

Nice To Haves

• Associate degree in a related field.
• Basic knowledge of adult learning models.
• Skill using electronic health records, such as EPIC, as applicable to the area of work.

Responsibilities

• Screen and assess patients for social determinants of health needs and connect them with community resources.
• Provide relevant community and health care resources and assist patients with navigating access to those resources when indicated.
• Document patient care in EPIC and follow workflows that capture activities and meet APCM patient engagement standards.
• Support CareSTEP activities, including online portal engagement, accessing community resources or services, transportation assistance, and coordinating care.
• Advocate for patients and teach strategies for self-advocacy.
• Table at community events to provide information about La Clinica services.
• Occasionally meet patients in the community to provide additional support accessing needed resources or services.
• Provide updated resource information to the Community Resource Team Lead.
