Requirements
- Active, current, unrestricted Physical Therapist (PT) license in the state of practice
- Graduate of an accredited Physical Therapy program (CAPTE accredited)
- Strong clinical, communication, and organizational skills
- Commitment to patient-centered, compassionate care
Nice To Haves
- Experience in skilled nursing, rehabilitation, or post-acute care preferred
Responsibilities
- Perform thorough evaluations, assessments, and treatments for patients recovering from illness, injury, or surgery
- Develop and implement individualized care plans and therapeutic interventions to improve patient mobility and function
- Collaborate closely with physicians, nurses, and other rehabilitation team members
- Educate patients and families on therapy plans, mobility techniques, and home exercise programs
- Accurately complete all required documentation and patient records in a timely manner
- Ensure care and services meet regulatory, facility, and professional standards
- Participate in quality improvement initiatives and staff in-services
