About The Position

Requirements

• Active physical therapy license in Pennsylvania (or eligible state per licensure requirements)
• Doctor of Physical Therapy (DPT) degree
• Recent, relevant home health experience
• EPIC experience and OASIS proficiency required
• Ability to perform Start of Care and revisits in a home health setting
• Strong communication skills and ability to work independently in patients’ homes

Responsibilities

• Provide physical therapy services to home health patients per physician orders, including initial assessment and ongoing visits
• Develop and administer patient-specific treatment plans to improve mobility, strength, balance, and functional independence
• Conduct Start of Care (SOC) and revisits, document progress, and adjust interventions as needed
• Complete documentation in EPIC and perform OASIS assessments as required
• Collaborate with the interdisciplinary team and instruct caregivers on home exercise programs and safety
