About The Position

Occupational Therapist (OT) – Home Health. BAYADA Home Health Care is seeking a compassionate and motivated Occupational Therapist (OT) to provide one-on-one home health visits to adult and geriatric patients throughout Monmouth, Middlesex, Somerset, and Union Counties. As a Home Health Occupational Therapist, you'll have the opportunity to make a meaningful impact by helping patients safely regain independence in the comfort of their own homes. Requirements

  • Current Occupational Therapist (OT) license in the State of New Jersey
  • Minimum of one year of recent Occupational Therapy experience
  • Graduate of an accredited Occupational Therapy program
  • Strong clinical assessment and critical thinking skills
  • Excellent communication, organization, and time management abilities
  • Comfortable using electronic medical records (EMR)
  • Ability to work independently while collaborating with an interdisciplinary team
  • Reliable transportation and willingness to travel throughout Monmouth, Middlesex, Somerset, and Union Counties

Nice To Haves

  • Home Health experience preferred, but not required

Responsibilities

  • Provide occupational therapy services to adult and geriatric patients in their homes
  • Complete comprehensive evaluations of patients' functional abilities, including: Activities of Daily Living (ADLs), Instrumental Activities of Daily Living (IADLs), Strength, endurance, coordination, balance, and mobility, Cognitive, psychological, sensory, and motor function
  • Develop individualized treatment plans designed to maximize independence and safety
  • Assess home environments and recommend adaptive equipment and environmental modifications
  • Implement therapeutic interventions that improve function and quality of life
  • Collaborate with nursing and the interdisciplinary care team to coordinate patient care
  • Educate patients, caregivers, and family members on therapeutic techniques, home safety, and strategies to promote independence
  • Reassess patient progress and modify treatment plans as appropriate
  • Complete accurate and timely electronic documentation using an EMR system
