About The Position

Requirements

• Current RN licensure from the Massachusetts Board of Registration in Nursing.
• At least two years of recent experience providing direct care to elders or adults with disabilities.
• Willingness to travel within the assigned region.
• Valid driver’s license and reliable transportation.
• Strong communication, documentation, and organizational skills.
• Solution-oriented approach and ability to multitask.

Nice To Haves

• Fluency in English and a second language is strongly preferred.
• Experience in case management, behavioral health, social services, and/or as a community health worker.

Responsibilities

• Complete intake, annual assessment, and admission visits.
• Complete required documentation, including client assessment forms and multidisciplinary plans of care, and communicate professionally with clients’ primary care providers and other healthcare providers when necessary.
• Review the client’s admission binder, confirm understanding, and educate caregivers on using the required caregiver communication tool or electronic visit verification system, as applicable.
• Conduct regular in-home visits, provide caregiver education and skills training, and assess health and environmental changes.
• Conduct semi-annual visits; review and update plans of care and goals; review and reconcile medications; and review caregiver communication tools.
• Address concerns related to the client’s environment, psychosocial status, and changes in physical health.
• Participate in multidisciplinary team meetings and collaborate to address client needs.
• Perform other duties within the scope of practice as assigned by management.
• Foster a culture of customer service and commitment to quality care.
