About The Position

Vitra Health is seeking a Registered Nurse for its Boston Region to support clients through home- and community-based services across Massachusetts. The role combines intake, assessment, and admission visits with non-clinical support, client and caregiver education, healthcare navigation, and assistance accessing community resources. The nurse helps clients remain safely in their homes while delivering compassionate, personalized care and promoting Vitra Health’s mission and values.

The position includes regular in-home and community visits, assessment and care-plan documentation, medication review and reconciliation, caregiver education, and collaboration with primary care providers and multidisciplinary teams. The nurse must travel within the assigned region and work in client homes and community settings, as well as perform computer-based documentation. Work may involve varying temperatures, odors, allergens, pets, and other environmental factors.

Requirements

• Current RN licensure from the Massachusetts Board of Registration in Nursing.
• At least two years of recent experience providing direct care to elders or adults with disabilities.
• Willingness to travel within the assigned region.
• Valid driver’s license and reliable transportation.
• Strong communication, documentation, and organizational skills.
• Solution-oriented approach and ability to multitask.

Nice To Haves

• Fluency in English and a second language is strongly preferred.
• Experience in case management, behavioral health, social services, and/or as a community health worker.

Responsibilities

• Complete intake, annual assessment, and admission visits.
• Complete required documentation, including client assessment forms and multidisciplinary plans of care, and communicate professionally with clients’ primary care providers and other healthcare providers when necessary.
• Review the client’s admission binder, confirm understanding, and educate caregivers on using the required caregiver communication tool or electronic visit verification system, as applicable.
• Conduct regular in-home visits, provide caregiver education and skills training, and assess health and environmental changes.
• Conduct semi-annual visits; review and update plans of care and goals; review and reconcile medications; and review caregiver communication tools.
• Address concerns related to the client’s environment, psychosocial status, and changes in physical health.
• Participate in multidisciplinary team meetings and collaborate to address client needs.
• Perform other duties within the scope of practice as assigned by management.
• Foster a culture of customer service and commitment to quality care.
