About The Position

BrightStar Care is seeking an experienced Registered Nurse with a strong wound care background to provide specialized home-based services in Humble, Kingwood, and nearby communities. The role provides wound, skin, ostomy, and continence care in patients’ homes, with a focus on promoting healing, preventing complications, and improving quality of life. The nurse works independently in a home care environment and collaborates with physicians, caregivers, and interdisciplinary care teams to support patients throughout recovery.

Requirements

• Registered Nurse (RN).
• Strong wound assessment and treatment experience.
• Excellent communication and documentation skills.
• Comfort working independently in a home care environment.
• Commitment to high-quality, patient-centered care and positive outcomes.

Nice To Haves

• Wound, Ostomy, and Continence Nursing (WOCN) certification.

Responsibilities

• Conduct comprehensive patient assessments.
• Develop individualized care plans.
• Perform wound treatments and dressing changes.
• Monitor patients’ healing progress.
• Educate patients and caregivers about prevention and ongoing wound management.
• Collaborate with physicians, caregivers, and interdisciplinary care teams.
• Document care using electronic charting.
