About The Position

Corewell Health operates more than 300 outpatient clinics across Michigan, including over 40 types of primary care and specialty practices. This role provides education, follow-up, and emotional support to people with intellectual and developmental disabilities and their families. The Nurse Navigator case manages the population across inpatient and outpatient settings and measures outcomes to support continuity of care. The position also manages clinical management projects and programs and conducts quality improvement activities to measure program processes and outcomes.

Requirements

• Bachelor's degree in Nursing.
• Five years of previous nursing or CDE clinical experience.
• Michigan Registered Nurse (RN) license required upon hire.
• Basic Life Support (BLS) certification from the American Heart Association or American Red Cross required within 90 days.
• Advanced Cardiovascular Life Support (ACLS) certification from the American Heart Association or Pediatric Advanced Life Support (PALS) certification from the American Heart Association required within 120 days.

Nice To Haves

• Master's degree in Nursing.
• Prior teaching experience with patients of all ages.
