About The Position

Requirements

• California-licensed Nurse Practitioner.
• At least 1 year of primary care experience.

Responsibilities

• Obtain patient histories and perform physical examinations.
• Review medications and provide related instructions.
• Review home safety and psychosocial needs.
• Fulfill documentation needs for STAR and HCC measures.
• Document pertinent information in patient charts and case management notes on the same day the patient is seen, including medications, end-of-life preferences, and drivers of preventable hospital admissions.
• Instruct patients, caregivers, and family members to contact the Nurse Practitioner before accessing medical care, except in life-threatening emergencies.
• Maintain patient safety and confidentiality.
• Use universal precautions in all patient care.
• Prescribe and order procedures, treatments, and medications within the scope of practice.
• Develop a high level of trust and intimacy with patients and families.
