About The Position

As a CenterWell Home Health nurse, reporting to the Clinical Manager, you provide personalized, one-on-one skilled nursing care to patients in their homes under the direction of an RN. You work with physicians and clinicians to manage care plans that support recovery and help patients regain independence. The role includes implementing individualized care plans with patients, families, and the healthcare team; educating patients and families on disease management, treatment options, and self-care; maintaining accurate records of visits, interventions, and outcomes; and promptly communicating patient or family needs and status changes to patients, physicians, clinical managers, and other care team members. You also participate in care coordination and discharge planning. This is a patient-facing position subject to Humana’s TB screening program. It is also subject to the driver safety program, requiring a valid state driver’s license and personal vehicle liability insurance. Candidates applying from a Florida work location are subject to Florida Level 2 background screening.

Requirements

  • Current nursing license in the practicing state.
  • Valid driver’s license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • Two years of experience as an LPN/LVN in a clinical setting.

Nice To Haves

  • Nursing experience in a Home Health or Hospice setting.
