About The Position

Brighton/Suncrest Hospice is seeking a Registered Nurse to provide compassionate, individualized hospice care wherever patients call home, including private residences, skilled nursing facilities, and assisted living communities. The RN collaborates with an interdisciplinary team of nurses, Social Workers, Chaplains, CNAs, and volunteers to develop care plans that address patients’ and families’ physical, emotional, and spiritual needs. Lower clinician caseloads and higher staff-to-patient ratios allow nurses to spend meaningful time with patients and families and provide personalized care. Various nursing opportunities are available depending on the RN’s strengths and interests.

Requirements

• Arizona Fingerprint Clearance Card.
• Current Registered Nurse (RN) license in good standing in the state of practice.
• Passion for providing exceptional, patient-centered hospice care.
• Strong collaboration and communication skills with interdisciplinary care teams.
• Ability to work independently while effectively managing a daily schedule.
• Excellent time management and organizational skills to balance patient visits, documentation, meetings, and urgent needs.
• Comfort learning and using electronic medical record (EMR) systems.
• Reliable transportation, a valid driver’s license, and current auto insurance to travel throughout the service area.

Responsibilities

• Provide individualized hospice care in patients’ homes, skilled nursing facilities, and assisted living communities.
• Collaborate with interdisciplinary team members to develop care plans supporting the physical, emotional, and spiritual needs of patients and families.
• Manage a daily schedule balancing patient visits, documentation, meetings, and urgent needs.
• Travel throughout the service area to provide patient care.
