Requirements

• Successful completion of an approved nurse practitioner program.
• Current approval by the Board of Nursing to perform medical acts.
• Current registered nurse license in the state.
• Current active, unrestricted APRN license in the state where the applicant applied.
• Master’s degree in Nursing specializing in Nurse Practitioner programs.
• Able to stand and walk for more than four hours.
• Able to lift 100 pounds to assist fallen residents and 10 pounds for routine essential functions.
• Able to push or pull more than 70 pounds when administering patient care.
• Able to bend, squat, kneel, and twist when administering patient care.
• Hearing sufficient to perform physical assessments; hearing aids are permitted.
• Vision sufficient to perform physical assessments; glasses or contacts are permitted.
• Depth perception sufficient to perform medical procedures.
• Fine motor skills sufficient to grasp and control medical equipment and perform precise procedures such as sterile dressing changes.
• Able to meet the regional travel requirement of at least four days per week.

Responsibilities

• Float within a designated region to cover schedules, fill in for local teams, and potentially provide care at consistent facilities.
• Plan, direct, and evaluate resident and employee health care in long-term care settings.
• Examine and treat chronic and acute episodic illnesses, minor injuries, accidents, and other injuries or illnesses.
• Perform selected laboratory testing, offer referrals, write prescriptions, and perform bedside procedures within applicable expertise, protocols, equipment, and scope of practice.
• Work with the medical director in the care and treatment of residents and staff.
• Debride wounds using sharp, enzymatic, autolytic, or surgical methods that can be safely completed at the patient’s location, including associated procedures.
• Apply and manage topical wound care and associated wound-management products.
• Direct and recommend services to improve wound-healing outcomes, including testing, referrals, home health care, Vacuum Assisted Closure devices, and orthotics.
• Interpret diagnostic tests and recommend appropriate follow-up.
• Recommend required medications, durable medical equipment, or supplies for wound healing or prevention within the practitioner’s scope, license, and collaborative agreement.
• Conduct diabetic foot-ulcer risk assessments for members with diabetes according to ADA guidelines.
• Educate patients about topical and systemic wound management.
• Educate primary care physicians, case managers, and referring physicians about current wound-management practices.
• Provide regular updates to primary care physicians, case managers, or referring physicians on patients’ systemic conditions associated with wound care.
• Serve as a resource for decisions, planning, and policies affecting residents’ health.
• Collaborate with staff to identify residents’ needs and help develop plans to meet those needs.
• Travel regionally at least four days per week, with possible last-minute coverage travel and overnight stays.
