About The Position

Requirements

• Graduation from an accredited LVN school
• Current LVN license issued by the State of California Vocational Nursing and Psychiatric Technicians
• A minimum of one (1) year of experience practicing as a Licensed Vocational Nurse
• At least one year of working with a frail or elderly population
• Valid California driver’s license with a good driving record
• A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines

• Experience and knowledge regarding the physical, mental, and social needs of the patients

Responsibilities

• Development and implementation of homecare services for program participants
• Coordination of all contracted home services
• Coordination of durable medical equipment
• Coordination of nursing home care
• Attending scheduled IDT and Coordination meetings
