About The Position

The Nurse Coordinator serves as a leader, change agent, and coordinator of patient care services. The role takes a lead role in designing and implementing a patient-centered care delivery system that facilitates patient outcomes, supports continuity of care, and enhances patient and caregiver satisfaction. The Nurse Coordinator is accountable to patients and families for the processes and outcomes of care during an episode of care, to peers for advancing group practice, and to the supervisor for overall work performance. Other duties may be assigned.

Requirements

• Graduation from an accredited school of nursing.
• A Bachelor of Science in Nursing (BSN) degree or enrollment in a BSN program is required; a BSN is preferred.
• Current licensure as a Registered Nurse in the State of Maryland.
• At least three years of relevant clinical work experience.
• Ability to respond to, investigate, identify, and problem-solve administrative and clinical matters requiring immediate and decisive action.
• Ability to use computer systems to manage patient information.
• Knowledge and skills to provide care appropriate to the ages of patients served, including understanding growth and development across the lifespan and assessing and meeting age-specific patient needs in accordance with department policies and procedures.
• Highly effective interpersonal, written, and verbal communication skills.
• Current CPR certification.

Responsibilities

• Be accountable to patients and families for care processes and outcomes during an episode of care.
• Advance group practice in collaboration with peers.
