About The Position

COPC is seeking an experienced full-time licensed board-certified NP/PA to join their team at Stonegate Family Health. This well-established practice focuses on health education, preventive medicine, and disease management to deliver high-quality patient care. The practice is located in Reynoldsburg, OH, approximately twelve miles from downtown Columbus.

Requirements

  • Master’s degree in nursing or physician studies from an accredited program.
  • Current licensure as a Nurse Practitioner in Ohio.
  • National certification as a Family Nurse Practitioner (FNP).
  • Strong clinical assessment, diagnostic, and treatment skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and collaboratively within a multidisciplinary team.

Nice To Haves

  • A minimum of 1 year of practical working experience as an NP post-graduation in an outpatient practice setting.
  • EPIC EHR experience is helpful.

Responsibilities

  • Conduct thorough assessments, including medical history reviews and physical examinations, to diagnose and treat acute and chronic health conditions in patients of all ages.
  • Provide a full spectrum of primary care services, including well visits, health promotion, disease prevention, routine screenings, vaccinations, and management of common illnesses and injuries.
  • Develop individualized treatment plans for patients with chronic diseases such as diabetes, hypertension, asthma, and COPD. Monitor disease progression, adjust treatment regimens, medication management and educate patients on self-management strategies.
  • Empower patients and families by providing comprehensive education on health maintenance, disease prevention, and treatment options. Address questions, concerns, and health-related inquiries to promote informed decision-making and adherence to care plans.
  • Collaborate with physicians, advanced practice providers, nurses to coordinate patient care across multiple disciplines and healthcare settings. Facilitate referrals to specialists and other healthcare providers as needed.
  • Maintain accurate and timely documentation of patient encounters, treatment plans, and follow-up care using electronic health record systems in compliance with regulatory standards and organizational policies. COPC uses EPIC.
