Nurse Practitioner (NP) Hospice
About The Position
Join Agape Care Group as a hospice and palliative care nurse practitioner, caring for seriously ill patients to alleviate burdensome symptoms and align medical treatments with patients’ goals and values. Work collaboratively with clinicians and teammates to provide high-quality care wherever patients call home. As an active member of the medical community in your territory, provide ongoing palliative care, hospice services, and education. The role offers supportive leadership, professional development opportunities, and room for advancement. Pay is determined by years of experience and location.
Requirements
- A heart to serve patients and families and a passion for providing the best possible care.
- Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing preferred.
- Two years of experience within the last five years in chronic disease management or long-term care.
- Reliable transportation.
- Ability to sit, stand, bend, and move intermittently; lift at least 80–100 lbs; and effectively bear the weight of an average adult.
