About The Position

Ohio’s Hospice is seeking a compassionate and experienced Registered Nurse (RN) to join our team as a Part-Time Triage Nurse. This role is ideal for an RN who enjoys providing clinical support, coordinating patient care, and helping patients and families receive timely, compassionate service during their hospice journey. The Triage Nurse – RN provides clinical support to hospice patients and families through our Call Center. You will assess incoming calls, prioritize patient needs, coordinate appropriate clinical services, and communicate with members of the interdisciplinary team to ensure continuity of care. Your clinical judgment and communication skills will help ensure patients receive timely care focused on comfort, dignity, and quality of life.

Requirements

  • Current Registered Nurse (RN) license in the State of Ohio
  • Current CPR certification
  • Valid driver’s license with a safe driving record
  • State-required minimum auto insurance coverage
  • Strong documentation and computer skills
  • Excellent assessment and clinical judgment skills
  • Pain and symptom management experience
  • IV and vascular access device skills
  • Ability to perform basic mathematical calculations, including whole numbers, fractions, decimals, ratios, rates, and percentages

Nice To Haves

  • CHPN certification required or must be obtained within two years of employment

Responsibilities

  • Receive and triage patient and family calls based on clinical urgency and need.
  • Provide professional, compassionate customer service and clinical guidance.
  • Assess patient needs and coordinate appropriate services according to the plan of care.
  • Communicate changes in patient condition or needs to the appropriate clinical staff.
  • Coordinate changes in the level of care with schedulers and other departments as needed.
  • Document assessments, interventions, observations, and communications accurately and promptly in the electronic medical record.
  • Maintain accurate call logs and ensure information is communicated effectively to the patient’s interdisciplinary team.
  • Support continuity and coordination of care to promote comfort and a peaceful end-of-life experience.
