About The Position

Requirements

• Current, unencumbered Registered Nurse (RN) license in the state(s) of practice, including eligibility for Nurse Licensure Compact (NLC) privileges where applicable.
• At least 1 year of experience as a Registered Nurse.
• Knowledge of the physical, psychosocial, and spiritual needs of terminally ill patients and their caregivers.
• Comfort and proficiency with technology, including electronic health records, email, internet navigation, and Microsoft Office applications.
• Ability to conduct effective clinical assessments and make sound nursing judgments via telephone.
• Willingness and ability to obtain additional RN licenses in other states as required; additional licenses are reimbursed by the employer.
• Solid critical-thinking, multitasking, and communication skills, including the ability to convey empathy and professionalism in high-stress situations.
• Reliable high-speed internet access for remote work.
• Ability and willingness to travel as required, with a valid driver’s license, reliable transportation, and required automobile insurance.
• Pass a drug test before beginning employment.

Nice To Haves

• At least 1 year of hospice nursing experience.
• At least 1 year of telephone or remote triage experience.
• Spanish-speaking proficiency.

Responsibilities

• Respond promptly to after-hours hospice calls, assess caller needs, and use professional nursing judgment to determine appropriate support and next steps.
• Conduct skilled telephonic assessments of patient, caregiver, environmental, emotional, and psychosocial factors using the plan of care and electronic health record.
• Provide patient and caregiver education, hospice guidance, and symptom management support consistent with agency resources and recognized standards of practice.
• Coordinate with on-call field staff, pharmacies, DME vendors, and other agencies to facilitate visits, follow-up, equipment needs, and continuity of care.
• Document assessments, interventions, communications, and care coordination activities accurately and in real time while complying with organizational policies and regulatory requirements.
• Perform other related duties as assigned.
