About The Position

Haven Hospice is seeking a Hospice Triage Registered Nurse to provide after-hours telephone support to hospice patients and families. The role is based in Gainesville and is PRN, with weeknight evening, overnight, and weekend shifts available. The nurse must pick up at least two shifts per biweekly pay period. The position supports patients from home while coordinating with a dedicated care team and is subject to a Level 2 background screening.

Requirements

• Graduate of an accredited nursing school and current licensure to practice in the state of operation.
• At least 1 year of hospice nursing experience.
• Nursing Compact RN license.
• Licensed driver with an insured automobile in good working order, consistent with state or organizational requirements.
• Excellent clinical assessment skills.
• Excellent observation, verbal and written communication, critical thinking, and basic math skills; nursing skills consistent with the competency checklist.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Able to pick up a minimum of 2 shifts per biweekly pay period.

Nice To Haves

• Three or more years of hospice nursing experience preferred.
• Experience in hospice or a similar setting preferred.
• Prior triage experience preferred.

Responsibilities

• Answer hospice calls during nights, weekends, holidays, and emergencies, including severe weather.
• Help schedule nurse visits, handle emergency patient admissions, and dispatch on-call staff.
• Assess patient and caregiver needs over the phone and provide support.
• Communicate patient updates to physicians and obtain new care orders when needed.
• Provide instructions and guidance based on each patient’s care plan.
• Coordinate after-hours equipment, supplies, and medication needs.
• Work with facility staff to support patients during off-hours.
• Document calls, care updates, and decisions clearly and accurately.
• Ensure care and procedures follow state and federal requirements.
• Maintain and develop professional knowledge and practice skills through continuing education and in-service classes.
