About The Position

Haven Hospice is seeking a compassionate, experienced Triage Registered Nurse to provide after-hours support for hospice patients and families. The role works from home and provides calm guidance, care coordination, and support during critical moments. The position is PRN, with weeknight evening, overnight, and weekend shifts available. Employees must be able to work a minimum of two shifts per biweekly pay period. The position is based in Gainesville and is subject to a level 2 background screening.

Requirements

• Graduate of an accredited nursing school and hold current licensure to practice in the state of operation.
• At least 1 year of hospice nursing experience.
• Nursing Compact RN license required.
• Licensed driver with an insured, well-maintained automobile meeting state or organization requirements.
• Excellent clinical assessment skills.
• Excellent observation, verbal and written communication, critical thinking, and basic math skills; nursing skills as specified by the competency checklist.
• Maintain and develop professional knowledge and practice skills through continuing education and in-service classes.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.

Nice To Haves

• Three or more years of hospice nursing experience preferred.
• Experience in hospice or a similar setting preferred.
• Prior triage experience preferred.

Responsibilities

• Answer hospice calls during nights, weekends, holidays, and emergencies, including severe weather.
• Help schedule nurse visits, handle emergency patient admissions, and dispatch on-call staff.
• Assess patient and caregiver needs and provide support by phone.
• Communicate patient updates to doctors and obtain new care orders when needed.
• Provide instructions and guidance based on each patient's care plan.
• Coordinate after-hours needs such as equipment, supplies, and medications.
• Work with facility staff to support patients during off-hours.
• Document calls, care updates, and decisions clearly and accurately.
• Ensure care and procedures follow state and federal rules.
