About The Position

Heart to Heart Hospice provides compassionate care to patients with life-limiting illnesses and their loved ones. The Triage Registered Nurse provides telephonic support to hospice patients and families during after-hours, weekends, when the office is closed, or at the request of the Executive Director. The role also coordinates after-hours visits, manages referrals, completes documentation, provides education and support, and conducts patient visits as needed. The position supports continuity of care and compliance with applicable laws, regulations, and agency policies.

Requirements

Current state Registered Nurse license.
Current state driver's license, reliable transportation, and ongoing valid and current auto liability insurance.

Nice To Haves

Two years of experience as an RN in a healthcare or clinical setting is preferred.

Responsibilities

Provide telephonic triage to hospice patients and their families during after-hours, weekends, when the office is closed, or at the request of the Executive Director.
Ensure after-hours visits are scheduled promptly with the on-call team.
Complete and submit documentation to the site at the end of each shift to ensure continuity of care for the weekday team.
Manage referrals received over weekends and after-hours.
Provide education and support to patients and families or caregivers.
Conduct patient visits as needed, including admissions, routine visits, pronouncements, and tuck-ins.
Promote positive, respectful communication with staff, referral sources, patients, and families.
Perform other duties as required.
Ensure overall compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures.
