About The Position

Desire Home Care, Inc is seeking a compassionate, detail-oriented LVN Case Manager to support patient-centered home health services in Riverside and San Bernardino Counties. This primarily office-based role supports RN Case Managers and field clinicians by coordinating care, managing visit frequencies and orders, triaging calls, and ensuring timely, compliant documentation under RN oversight. Occasional field ride-alongs or visits may occur as needed.

Requirements

  • Graduate of an accredited practical/vocational nursing program with an active, unrestricted California LVN/LPN license and current BLS/CPR certification.
  • Minimum one year of recent LVN nursing experience.
  • Working knowledge of home health requirements, including PDGM, LUPA, homebound/skilled need criteria, OASIS-E terminology, Medicare Conditions of Participation, and LVN scope of practice.
  • Proficient with home health EMR systems, computer documentation, phone triage, and SBAR communication.
  • Solid wound care knowledge and ability to support chronic disease education, including diabetes/insulin, CHF/COPD, and hypertension.
  • Strong written/verbal communication, organization, follow-through, independent work skills, and time management under RN supervision.
  • Valid California driver’s license, reliable transportation, required automobile insurance, vehicle in good working order, current physical, and TB clearance.

Responsibilities

  • Coordinate day-to-day patient care from the office, including triaging calls, supporting field staff, and communicating changes to providers and RN Case Managers using SBAR.
  • Plan and monitor visit frequencies per the RN plan of care; track utilization, PDGM, and LUPA risk; and recommend adjustments as needed.
  • Process, enter, and track provider orders; obtain timely signatures and close the loop within 24 hours.
  • Review EMR dashboards for expiring frequencies, unsigned notes/orders, labs/results, ED visits, and hospitalizations; escalate concerns promptly.
  • Prepare for and participate in weekly interdisciplinary case conferences; update goals, risks, and action items.
  • Support medication reconciliation, refill coordination, patient/caregiver teaching, wound care coordination, and chronic disease education by phone.
  • Support intake, transitions, and discharge planning by verifying eligibility, homebound status, payer requirements, authorizations, and pre-visit needs.
  • Perform light QA audits for skilled need documentation, teach-back education, timely notes, and staff corrections.
  • Maintain compliance with Medicare Conditions of Participation, California LVN scope of practice, HIPAA, infection control, safety standards, and agency policies.
  • Provide skilled nursing care, documentation, supervisory visits, and other job-related duties within LVN scope as assigned.
