About The Position

Lorian Health is hiring a Hospice Licensed Vocational Nurse in Las Vegas, Nevada, serving the Northeast, Northwest, and Southeast areas. The role provides skilled nursing care in patients’ homes in accordance with the RN-established Plan of Care, physician orders, and the LVN scope of practice. The position uses a pay-per-visit compensation model, with visit rates from $33 to $42 depending on experience and visit type. The schedule is Monday through Friday, with rotation for on-call weekends.

Requirements

Certified or licensed to practice in the state of practice and in good standing.
Graduate of an approved school of practical or vocational nursing.
At least one year of recent experience, within the last three years, in a Home Health or Hospice setting.
Current CPR certification; BLS is required.
Valid driver’s license.

Responsibilities

Provide skilled nursing care in accordance with the RN-established Plan of Care, physician orders, and applicable LVN scope of practice.
Monitor and assess patient condition, identify changes or concerns, and promptly communicate updates to the patient, family or caregiver, physician, and supervising nurse.
Deliver safe, high-quality home health services while assisting patients with activities of daily living and promoting independence and self-sufficiency.
Educate patients, families, and caregivers on medications, treatments, health management, and self-care.
Maintain accurate, timely, and thorough clinical documentation in accordance with Lorian policies, procedures, and regulatory requirements.
Coordinate patient care with the interdisciplinary team and identify and communicate the need for additional services such as PT, OT, ST, MSW, CHHA, RN, or dietary support.
Participate in case conferences and ongoing care coordination.
Use supplies and equipment efficiently while maintaining a patient-centered approach to care.
